Understand Coverage
Restoration progress is often described with coverage index levels. Your starting level helps determine whether manual methods, T-tape, or devices make the most sense.

Tight cuts may need manual methods or T-taping first. Once enough slack is available, devices will maximize rate of progress.

Manual Methods
Hand-applied stretches can control tension directly and cost nothing. They are useful for beginners, tight starting points, and targeting specific areas.
BEST FOR CI-0 and CI-1 STARTS
T-Tape
Fabric medical tape can be tensioned with straps to pull skin forward. T-tape can be more practical for some beginners and can help create enough slack to progress.
Often around CI-2 and above
Dual Tension Devices
Devices such as the DTR and Mantor Flow use a pusher and gripper system to exert force in opposing directions, and rods are lengthened as progress increases. Additional stimulus can be applied to the system with strap or weight tension.

Inflation Devices
Inflation devices create outward tension from within and can be paired with strap tension. Soft grippers and uniform expansion may allow longer sessions for some restorers.
Routine
Consistency beats intensity.
Restoration works best as a consistent daily practice. Aim for sustainable sessions, comfortable tension, and enough rest for skin to recover.
- Aim for a 2-3 day tugging block with sessions totaling ~8 hours per day.
- Take a rest day between each tugging block or when skin becomes irritated/sore.
- During rest days or sleep wear retainers to train skin forward and revitalize glans.
- Moisturize after tugging and keep devices clean for hygiene and grip.
Expectations
Common questions before starting.
Does it hurt?
The stretching doesn't hurt and becomes mostly unnoticeable during sessions. Only towards the end of a tugging session on some devices, gripper pinching fatigue becomes noticeable.
How long will it take to see results?
A general rule of thumb is to gain 2 coverage index levels in the first year, and then continue to gain 1 CI level per year subsequent. Most restorers aim to reach flaccid coverage which can take a few years, but much of the benefits can be attained more quickly.
Will inflation cause a loose foreskin?
When inflation devices first gained popularity, there was logical concern that outward expansion would cause a loose/baggy foreskin. It was soon realized that regardless of method the restored foreskin naturally settles and contracts around the shape of the glans because the new tissue includes dartos fascia muscle.
Are results permanent?
Yes, the tissue growth and restored foreskin are permanent, but because it lacks the ridged band of intact foreskin, it may not always remain fully forward and is highly dependent on body position, activity level, and temperature. This can be addressed by retainer/O-ring use or restoring long enough to achieve flaccid overhang.
